Required Documentation for Appointment/Promotion at all Active Ranks
Documentation must be submitted as a full and complete packet to the Office of Faculty Affairs to be processed for action. Email a completed packet to faculty_affairs@rush.edu. Once submitted, it will be reviewed for requirements, accuracy, clarity, and the submitter will receive a response stating next steps. Complete packets will be directed to appropriate committee for reviews and approvals.
All packets are reviewed and approved by Faculty Council, Dean and Provost
Packets for senior appointments and promotions are first reviewed and approved by COSFAP and then by Faculty Council

- Chair Recommendation Letter (referencing appointment, term, rank, effective date) with chair signature
- Departmental Advisory Committee Approval (This can be included in the chair’s recommendation letter)
- Updated CV (ensure CV references active rank)
