As an integrated department within the College of Nursing, our Faculty Practice program matches expert faculty clinicians and nursing students with partner sites to deliver comprehensive nursing services to a variety of patient populations. Our partners include non-profit social service organizations, academic institutions, private health care providers and general businesses. Outcomes from our partnerships are outstanding and include improved health biometrics, decreased health care costs, and high partner and patient satisfaction.

The College of Nursing's Faculty Practice delivers nursing services and direct care to diverse populations at more than 20 on sites across Chicago. One of these sites is a clinic at St. Leonard's Ministries. St. Leonard's provides formerly incarcerated men and women with housing, employment and educational services to help people transition back into the community. The Faculty Practice clinic manages health needs and fill gaps as St. Leonard's residents work toward establishing primary care providers for the time when they leave.

Bobby E. Wright's mission is to improve the quality of life for Chicago’s Westside residents by providing the highest quality of mental health, substance abuse, and developmental disabilities services to children, adolescents, and adults within a cultural context that is sensitive to the community. These services are available to all persons who meet the admission criteria regardless of gender, sexual orientation, race, nationality, religion, etc. No one is denied services due to inability to pay.

Services Provided: outpatient therapy, case management, psychiatric evaluations, and establishing relationships with substance abuse centers to provide a warm handoff to improve the continuum of care for any members of Bobby E. Wright that we refer to various inpatient substance abuse programs located around Chicago.

Family Connects Chicago at RUSH is an in-home nurse service available to Chicago families with newborns. The service is simple: A registered nurse visits your home around 3 weeks after birth to check on mom, baby and the whole family. RUSH partners with the Chicago Department of Public Health to offer this program at no cost to families.
Site location: College of Nursing in Partnership with the Chicago Department of Public Health
Population served: Chicago families with newborns.
Services provided: Maternal and infant health assessment, and linkage to community resources to support newborn care as part of Family Connects Chicago.

The Community Health Nurse is accountable for coordinating and facilitating client services across the various levels of the continuum for a population of clients. In this role, Keith provides direct patient care through patient education and guidance regarding medications, lifestyle modifications, and treatment plans.

Heartland Alliance Health’s clinicians focus on care for members of the community who are experiencing homelessness or unstable housing. Some of my responsibilities include teaching and implementing infection control measures, COVID testing, providing education on disease prevention and chronic disease management, performing health screenings, and administering vaccinations.

North Side Housing and Supportive Services (NSHSS) is a housing program that has a 72-bed men’s emergency shelter in Uptown as well as over 170 permanent supportive housing units throughout Chicago for formerly homeless adults. We work with over 100 clients a year, providing ongoing in-home health education and medical case management for clients who are housed, in addition to consulting on the health needs of clients who are currently experiencing homelessness.

Services Provided: The Office of Student Health Services consists of health care practitioners that work in collaboration with Rush Employee and Corporate Health Services and Infection Prevention and Control Team. The Student Health team ensures the university’s COVID-19 policy and procedures are in alignment with that of the Rush System’s and keeps the health and safety of students and campus a priority. The Office of Student Health Services works collaboratively with each college to confidentially track and report student compliance for both COVID-19 and Influenza requirements, as well as manages the COVID-19 weekly testing requirement for students with an approved exemption. The Office of Student Health Services also collaborates with Infection Prevention and Control on student contact tracing and exposures to COVID-19 and serves as a resource to students for questions regarding required vaccinations, compliance, and protocols with returning to campus after testing positive for COVID-19.

Thresholds clinicians have a primary focus on Mental Health and work on the ACT (Assertive Community Treatment) team. ACT teams serve individuals struggling with a high acuity of mental health needs and our goal is to keep these individuals living in the community independently.

Population served: Adult psychiatric patients/dual Dx.
Services provided: Primary care and chronic disease management for adult men and women. Faculty Practice registered nurses provide integrated health care case management, individual therapy, health promotion, health screening activities and medication management for adults with serious mental illness.
