Located on Chicago's near west side in the Illinois Medical District, our clinic cares for residents of St. Leonard’s and Grace House who are usually formerly incarcerated individuals or have received recent treatment of substance use disorders. Our clinic manages chronic and episodic health needs and fill gaps as residents work toward establishing primary care providers for the time when they leave St. Leonard’s. In the case of extended incarcerations, we provide assistance for navigation of the current healthcare system, including: how to use insurance, fill prescriptions and steps to make routine appointments.
The College of Nursing's Faculty Practice delivers nursing services and direct care to diverse populations at more than 20 on sites across Chicago. One of these sites is a clinic at St. Leonard's Ministries. St. Leonard's provides formerly incarcerated men and women with housing, employment and educational services to help people transition back into the community. The Faculty Practice clinic manages health needs and fill gaps as St. Leonard's residents work toward establishing primary care providers for the time when they leave.
